Description of "dm: optimize dm_request_fn()"

The change “dm: optimize dm_request_fn()” introduces changes as follows.

dm: optimize dm_request_fn()

DM multipath is the only request-based DM target -- which only supports
tables with a single target that is immutable.  Leverage this fact in
dm_request_fn().
